Recent genetic studies of RA susceptibility have revealed the importance of the genes for cytokines, cytokine receptors, and immunoinflammatory regulators. Therefore, we investigated the association between pro- and anti-inflammatory cytokine/receptor gene polymorphisms and RA in a Korean population. Single nucleotide polymorphisms of the TNF gene (-308A/G, -238G/A, -489C/T, +252A/G), IL-1 receptor gene (IL-1RA, IL-1R1, IL-1R2), IL-6 (-174G/C), and IL-10 (-1082G/A, -819C/T) were analyzed by restriction fragment length polymorphism methods. The polymorphism of the IL-6 gene (rs1800795 C/T) was further investigated by the polymerase chain reaction-restriction fragment length polymorphism method. The mean age of the study subjects was 54.3+/-12.9 years (range, 14-79 years) and 72% were women. HLA-DRB1*04 was found in 31% of the patients with RA. Three (3.7%) of the 77 HLA-DRB1*04-positive patients were negative for rheumatoid factor. The heterozygous genotype of the IL-1RA gene (1168A/G) was significantly associated with RA susceptibility (OR, 2.46; p=0.03). The frequency of the IL-1RA genotype (1168AA) was significantly higher in DRB1*04 positive patients than in patients without DRB1*04 (p=0.04). Serum concentrations of TNF-alpha, IL-1beta, and IL-6 were positively correlated with the carriage of HLA-DRB1*04 gene (p=0.004, p=0.02, and p=0.05, respectively).
