Electron microscopical studies were made on the kidney in 170 patients with hyperlipoproteinaemia, of which 77 had nephrotic syndrome. Markedly increased numbers of lysosomes with lipoprotein material could be identified in the proximal tubular epithelial cells of patients with hyperlipoproteinaemia and nephrotic syndrome. In addition, lipoprotein was also observed in the macrophages of the mesangium and the arterioles. The mesangial cells of patients with nephrosis, on the other hand, had no evidence of increased numbers of lysosomes or of lipoprotein material, but in a subgroup of patients, the mesangial cells displayed decreased numbers of lipid droplets. Atrophic changes of the glomeruli were frequently found; however, these changes were not directly related to the nephrotic syndrome or to the presence of abnormal lipoproteins. Of the 170 patients with hyperlipoproteinaemia, 21 had mesangial hypercellularity of the glomeruli; this was invariably associated with proteinuria, but only in 1 patient did this change occur in the absence of nephrotic syndrome or hyperlipoprote
